Certified Home Health Aide (CHHA)
Personal Care – Assist patients with personal hygiene.
Light Housekeeping – Assists patient in meeting environmental needs such as changing linens, making the bed and light housekeeping.
Assist patients in and out of bed, and in ambulation.
Assist patients with prescribed exercises (taught by appropriate personnel).
Prepare meals, including therapeutic diets, and assists patients with eating.
Assists patients with medications which are ordinarily self-administered. The CHHA shall NOT administer medications of any kind.
Reports changes in the patients condition and needs to the supervising nurse or therapist.
Observes for non-compliance of home safety measurements by PT/PCG and reports such findings to the SN/therapist.

Adaptive Equipment – Instructs patients and their families in the use of prosthetic and orthotic or assistive devices such as canes, walkers, wheelchairs, and so forth.
Energy Conservation.
Fine Motor Skills.
Speech Therapy (ST)
Speech and language evaluation.
Development testing.
Swallowing Therapy.
Audiometry.
Other:
Medical Social Worker (MSW)
Counseling.
Evaluation of home environment.
Crisis Intervention.
Referral to community resources (In-Home Supportive Services, Transportation, Meals-On-Wheels)
Long Term care planning.
